【工管系公告】114-2學期-專業證照獎勵申請 Professional License Stipend– 2026 Spring (114-2) Semester

欲申請專業證照獎勵者（114年8月1日至115年1月31日核發且需為在學期間取得之證照），請於 3/6(五)下午5點前 繳交申請資料至工管系辦，逾期繳交或資料不全者將不予受理。

＊申請資料：

1. 申請表

2. 證照正/反面影本

3. 請在學習歷程平台登錄資料，並且列印影本

To apply for the Professional License Stipend (for licenses issued between August 1, 2025, and January 31, 2026, and obtained during your period of enrollment), please submit the required documents to the Department Office by 5:00 PM on Friday, March 6.  Late submissions or incomplete applications will not be processed.

* Required Documents:

1. Application form
2. Photocopy of the certificate, front and back
3. Register your certificate in the school system and print out the page information.
