欲申請專業證照獎勵者（114年2月1日至114年7月31日核發之證照），請於 9/30 (二)下午5點前 繳交申請資料至工管系辦，逾期繳交或資料不全者將不予受理。

1. 申請表

2. 證照正/反面影本

3. 請在學習歷程平台登錄資料(參考附件)，並且列印影本

To apply for the Professional License Stipend (for licenses obtained between February 1, 2025, and July 31, 2025), please submit the required documents to the Department Office by Friday, September 30.  Late submissions or incomplete applications will not be processed.

* Required Documents:

1. Application form
2. Photocopy of the certificate, front and back
3. Register your certificate in the school system and print out the page information.
